Is Notchplasty Essential for Arthroscopic Reconstruction of ACL



Abstract
The notchplasty is emphasized recently to prevent retear of the ACL caused by impingement. However, this is no doubt invasive procedure. Authors worried about adverse effect of this procedure which might cause excessive synovial reaction and
proliferation of healing tissue. In this study the results of ACL reconstruction without notchpoasty were analyzed. Sixty-four knees including 36 knees of rear entry technique and 28 knee of endoscopic technique were involved in this study. In
all
cases
the notch width indices were greater than 0.2. the follow-up period was 2.6 years in average (range : 1-4 years). Results were as follows ; 1) In no case retear was found. ; 2) Mild anterior laxity less than 3mm was found in 4 knees of rear entry
technique group and 3 knees of endoscopic technique group. However no case showed positive pivot shift test.; 3) The cyclops syndrome developed in 3 knees of rear entry technique group. However, their symptoms were too mild to perform surgical
correction. From this study it was suggested that a notchplasty should be performed in only selected cases because it did not seem to be essential procedure for the cases of normal sized notch.
